form completed with credit card information and signed
by you. We will also need a copy of your original prescription.

order is a refill
option. If you
do not have your
information online
with us, all
you need to do
is call us at

(toll free).
Monday to Friday
9AM to 5 PM Pacific
Standard Time.
No other information
is needed unless
there has been
a change in your
address, health
condition, in
the strengths
or quantities
of the medications
you are taking,
or in the method
of payment. Once
a prescription
has expired,
it will be necessary
to provide a
copy of the new
prescription
issued by your
doctor.

of residence. The prescription should be faxed along

Form and valid prescription. Allow up to 1 week
for shipping. Refill orders may be processed sooner,

medications when required.

in receiving

from you

your doctor
or pharmacy

copy of
the prescription.
B) The Order Form is inaccurate or incomplete. Your
order will be processed faster if you complete and
fax the Patient Information Form as soon as possible.
